OFFICE POLICY

This is a consultation practice. All patients are required to have a primary care provider whose name and number must be recorded on the medical history from with Dr. Trindade. All patients must have a yearly physical exam by their primary care provider and female patients must also have e yearly gynecological exam, a bone mineral density, and a mammogram at least every two years if indicated. Please bring copies of reports from your primary care provider to your visit with Dr. Trindade.

• Fee schedule

o Initial new patient @ 90 minutes: $675

o Follow up @ 60 minutes: $450

o Follow up @ 30 minutes: $225

§All fees are due at the time of service and payable by check, cash, or credit card. §Patients will receive a reminder phone call there to four days before their appointment.

INSURANCE POLICIES:

Dr. Trindade does not participate in any insurance programs. Consequently, you must pay for your visit at the time you are seen. We will provide you with detailed statement of services for you to submit to your insurance company for reimbursement. You may not be fully reimbursed. Your insurance pays for covered items and services per their contractual obligations according to your policy. Some services are considered preventative and are often not part of a policy. Laboratory, X-ray, prescription on other procedures are usually covered by your insurance. Dr. Trindade is no longer a Medicare provider and Medicare patients are not able to submit their superbill to Medicare since Dr. Trindade has ‘opted-out’ of Medicare.

CANCELLATION POLICY:

Please give us 24 hours notice if you need to cancel your appointment. After your first missed appointment, there will be a $25 fee charged for every additional missed appointment.

FACILITIES:

The office in Capitola has a remote receptionist who checks phone messages twice during the day, Monday through Friday, she also schedules appointments. Call and faxes received after hour will not be processed until the next business day. Dr. Trindade teaches for several different organizations and practices out of town at times, therefore it may be a few days before we can get back to you. This is not an urgent care practice, please use good judgement and seek care through your primary care physician or an urgent care facility if you have an urgent matter.

LOCATION:

From Highway 1 South take the State Park Exit and make a right at Soquel Dr by Safeway. Continue on Soquel Dr until you reach 9019 Soquel Drive—it will be on your left side. Parking is in front of the building. The office is on the building on the left when you go down the stairs.
From Highway 1 North take Rio Del Mar Blvd and make a right at the light. Then make an immediate Left onto Soquel Drive. Follow Soquel Drive to 9019—it will on your right. Park on the parking lot in front of the building. When you walk down the stairs it is the building on the Left.
